Do you get a badge/award or something for exploring them all?

Where are they all. I only seem to be able to find the ones connected with the missions. If there are 14 in Egypt I have no clue where some of them are.

They are all connected but the actual amount was less. When I was exploring a new tomb or had taken a quest, a single tomb could lead to other tomb and counted not as one but more than one. As I progress, I will unlock more and some requires special key which can be activated by accepting a quest. I won't progress much should I had declined any quest but some are not quest-related as I was able to explore freely.

I think that was how you will get 14 in Egypt for example and written in the journal. If you had explored by the actual amount, lets say 7, it might not be so interesting and the quest might ended sooner than what you had anticipated.
