I have a very thin male sim and I want him to get muscles. He has the athletic skill maxed out. Can he get there by continuing to work out and how long would it take?

He needs to use weights, not aerobic equipment.

Will it actually make the muscles show up? I read elsewhere that you can't change their bodies aside from CAS (and mine was born this way) and my skill is maxed out so I don't know if it will have an effect.

It will. If you want muscle tone to be gained, you have to download the dynamic sixpack mod by Consort.

You could also save up your lifetime points, there is a body changer reward that costs a lot, but you can make your sim thinner, fatter and more or less strong.
It won't change how defined the muscles are though, I think, unless it's been changed since I used it last.

If you want him to bulk up he's going to have to eat and put on weight as well.
