Do you want a cigarette or a cigar? Your thread title says cigarette but your post says cigar. If you mean cigarette, there's already an accessory cigarette that goes in the mouth: http://one-billion-pixels.blogspot....ccessories.html

If it's a cigar accessory you want, then to get started on making one yourself (requests to make things for you are not allowed on this site), you can take a look through the TS4 Tutorials section here. There are also accessory tutorials on the Sims 4 Studio site. Once you get started, if you have questions you can ask them in this forum or at the Sims 4 Studio site's Creator Help forum.
