Lothario comes from the name of a character used in the novel
Don Quixote. The Lothario name is also used in the play
The Fair Penitent. In Quixote, one of the characters named Anselmo becomes obsessed with testing the fidelity of his wife, and then goes out to seek help from his friend, Lothario, into seducing her. Hence the Lothario name existed way before the creation of The Sims 2 and isn't a name unique to the series, and why the name is associated with seduction of women.
Hence, Don (Quixote) Lothario.
Edit: In the image you displayed the name Lothario is being used to inquire about if the unnamed woman is married to the idea of being seduced by this "poor man". Not that the name of the poor man is actually Lothario.
