It's possible to make a custom sculpt and use it as a CAS preset which applies a morph to the base face. Since this is a face, you could modify the base face mesh using Milkshape to make a Blend Geometry (BGEO) morph, or any meshing program that uses OBJ format to make a DeformerMap (DMap) morph. You'd then need to clone a CAS Preset (CPRE) resource and a Sculpt (SCLP I think) resource, give them new instance IDs, and modify them to use your morph.
At this time you'd have to use my beta of TS4 Morphmaker to make the morph files and s4pe to do the cloning and linking. I've been poking along at a new version of Morphmaker that can do the whole package more easily and a set of tutorials, and I'll try to put a rush on it if you'd rather wait.
